Home and Community Based Services

ACT is a program of comprehensive outpatient services consistent with evidence-based practice standards published by the Substance Abuse and Mental Health Services Administration, provided in the community and directed toward the amelioration of symptoms and the rehabilitation of behavioral, functional, and social deficits of individuals with severe and persistent mental illness and individuals with complex symptomology who require multiple mental health and supportive services to live in the community.

IPR services are designed to facilitate a process of systematic change for individuals with serious mental illness, who want to make a change in one of the following environments: learning, working, living, or social engagement.  IPR’s focus is to promote recovery, full community integration, and quality of life through participation in weekly group and individual sessions.

Intensive residential service homes or intensive residential services meaning intensive, community-based services provided 24 hours a day, 7 days a week, 365 days a year to individuals with a severe and persistent mental illness who have functional impairments and may also have multioccurring conditions. Providers of intensive residential service homes are enrolled with Medicaid as providers of HCBS habilitation or HCBS intellectual disability waiver-supported community living and meet additional criteria specified in Iowa Code.

Peer Support Services are services provided by an individual who is personally living well in recovery from a serious mental illness, known as a peer support specialist. Peers provide support to other peers and assist them in reaching and maintaining their personal recovery goals.  Services are designed to promote skills for improving mental and physical wellbeing and increasing resiliency and self-determination.

Services provided in a noninstitutional setting to adult individuals with mental illness, intellectual or developmental disabilities, brain injury to meet the person’s daily living needs.

An approach to helping individuals participate as much as possible in competitive work in integrated work settings that are consistent with the strengths, resources, priorities, concerns, abilities, capabilities, interests, and informed choice of the individuals. Services are targeted for individuals with significant disabilities for whom competitive employment has not traditionally occurred; or for whom competitive employment has been interrupted or intermittent as a result of a significant disability including either individual or group-supported employment, or both, consistent with evidence-based practice standards published by the Substance Abuse and Mental Health Services Administration.
