Crisis Services

Access Center is the coordinated provision of intake assessment, screening for multi-occurring conditions, care coordination, crisis stabilization residential services, subacute mental health services, and substance abuse treatment for individuals experiencing a mental health or substance abuse crisis who do not need inpatient psychiatric hospital treatment, but who do need significant amounts of supports and services not available in other home and community-based settings.

Sioux Rivers provides funding to the Access Center located within Siouxland Mental Health as our contracted provider. Service Coordinators do not need to be contacted to access services, but many times they are contacted and will assist in determining the need for Access Center programming. The Access Center can be accessed by individuals, through other crisis services programming, or by local law enforcement as an alternative to a local ER (when no physical concerns are present).

Crisis residential 23-hour observation chairs and residential beds are alternatives to inpatient hospitalization when additional support is required for the management of mental health symptoms, but not to the extent that hospitalization needs to occur. To access these programs a crisis evaluation must support the need for services.

23 Hour Observation is a level of care provided for up to 23 hours in a secure and protected, medically staffed, psychiatrically supervised treatment environment.

Crisis Stabilization Residential Service (CSRS) are services provided in short-term non-community-based residential settings to de-escalate and stabilize a mental health crisis in an organizational setting of no more than 16 beds. Generally, crisis residential services are a 3–5-day service.

Sioux Rivers provides funding and support for three adult crisis residential programs. Warm handoffs to Sioux Rivers Service Coordinators assist the individual with follow-up services to ensure the continuation of care. Referrals for this program occur after a crisis evaluation is completed by access centers, mobile crisis providers, local hospitals, and outpatient therapy programs.

Children’s Residential Crisis Services are made available through Avera Behavioral Health in Sioux Falls, SD. To access these services first access Mobile Crisis and/or Sioux Rivers Service Coordinator.

Crisis Stabilization Community-Based Services (CSCBS) are voluntary services that provide supports to individuals and families within their own homes or designated places within the community. These supports include therapy and skill-building services designed to meet the individual’s immediate needs.

Sioux River provides funding to Seasons Center for Behavioral Health as our Crisis Stabilization Community Based Services provider in all six of our Regional Counties.

Mobile Response is a mental health service that provides on-site, face-to-face mental health crisis services for individuals experiencing a mental health crisis. Mobile crisis staff have the capacity to intervene, wherever the crisis is occurring, including but not limited to the individual’s place of residence, emergency rooms, police stations, outpatient mental health settings, schools, recovery centers or any other location where the individual lives, works, attends school, or socializes.

Sioux River provides funding to Seasons Center for Behavioral Health as our Mobile Crisis provider in all six of our Regional Counties. You do not need to contact a Service Coordinator to access Mobile Crisis, however, Service Coordinators are available to assist in any other service needs that might be identified as a result of crisis evaluation and support care.

Subacute services are a comprehensive set of wraparound services for persons who have had or are at imminent risk of having acute or crisis mental health symptoms that do not permit the persons to remain in or threatens the removal of the persons from their home and community, but who have been determined by a mental health professional and a licensed health care professional, subject to the professional’s scope of practice, not to need inpatient acute hospital services. A crisis evaluation must support the need for services. 

Sioux Rivers provides funding and support to Siouxland Mental Health for Subacute Care. For more information contact a Regional Service Coordinator.
